One Flew Over the Cuckoo's Nest

A 1975 American drama set on a psychiatric hospital ward, widely discussed in the history of institutional psychiatry, group meetings and patient autonomy.

Viewing note

This feature film is a fictional dramatisation adapted from Ken Kesey’s 1962 novel. It portrays an authoritarian mid-twentieth-century asylum regime as an allegory of institutional control and is not representative of contemporary mental health care or group psychotherapy.

Country: United States

Running time: 133 minutes (AFI catalogue)

What the work shows

Directed by Miloš Forman and filmed on location at Oregon State Hospital, the film follows Randle Patrick McMurphy (played by Jack Nicholson), who is transferred from a prison farm to a psychiatric hospital for evaluation. On the ward he clashes with the head nurse, Mildred Ratched (played by Louise Fletcher), over ward routines, daily group therapy meetings, medication and patients’ freedom to leave.

How closely it reflects practice

In the film, daily ward group meetings are depicted as coercive exercises in public humiliation rather than supportive therapeutic groups, and electroconvulsive therapy and lobotomy are depicted as punishments for defiance. Historians and psychiatrists have noted that while the film reflected 1960s and 1970s critiques of custodial asylums and involuntary institutional control, its portrayal of psychiatric treatments and staff shaped lasting public fear and stigma around inpatient care and group therapy.

The film has not been viewed for this draft; descriptions rely on the AFI catalogue and published reference accounts.

Limits of this entry

Production credits and running time come from the American Film Institute catalogue. This entry does not evaluate the history of physical treatments in psychiatry.
